On the “Images & Receipt” field, you will need to provide an archive (i.e. ZIP file) of at least 1 image of the affected item and a clear photo of the item’s purchase receipt. If you are unable to create a ZIP file / archive of the images, please submit the form once attaching the purchase receipt and then again, attaching the item image. Use the same contact details on both.
